Equipment Loan Program

Easter Seals Louisiana offers the loan of Durable Medical Equipment (DME) to disabled individuals who have no other resource for the equipment. Easter Seals maintains an inventory of donated new and used Durable Medical Equipment. Thanks to these generous donations Easter Seals is able to provide a much needed service in our community. As the Equipment Loan Program is indeed based on donated equipment, availability of certain items is limited. If a piece of equipment is unavailable and Easter Seals cannot fulfill your request immediately, a staff member will place your name on a waiting list, as well as search for alternative resources for your need. It is the program's goal to provide any disabled individual in need of equipment that will promote mobility, communication and greater independence for that person.

Durable Medical Equipment most commonly provided by Easter Seals includes standard and motorized wheelchairs, bedside commodes, shower and tub benches as well as transfer devices. Walkers, canes and reachers are also available. The process for obtaining a piece of equipment is simple. Contact our New Orleans office and ask to speak to someone about the equipment loan program. Someone will take your name, address and information concerning your request. You will be mailed a short application requesting more detailed information concerning the individual in need and what item he or she is requesting. Included in the application as required by HIPAA (Health Insurance Portability and Accountability Act of 1996), the recipient will also find a copy of Easter Seals Louisiana's Notice of Privacy Procedures and an accompanying signature page to be signed once the Notice of Privacy Procedures has been read. The individual in need will also need to provide a physician's prescription for the item, as documentation of medical necessity is required. Once the application has been completed, the Easter Seals Louisiana's Notice of Privacy Procedures form has been reviewed and signed, return the completed application, signature page and prescription to 305 Baronne Street, Suite 400; New Orleans, LA 70112-1617 or fax the documents to (504) 523-3465.
